The Twist-N-View™ range
The Twist-N-View™ range has been developed to meet the needs of those concerned with confidentiality. Not everyone has the space to put our larger charts on the wall in a manner to ensure that personal information is kept confidential. Twist-N-View Care Charts address both these issues: the charts are smaller and, in making them double-sided we have ensured that all personal information can remain confidential, yet all a carer needs to do is turn the chart over to access information that will help them provide informed, person-centred and dignified care.

The Image version
This Image version of the Twist-N-View™ range functions as an attractive picture to be hung on the wall near the individual’s bed, and has space for their needs and preferences to be written on the reverse. The charts have a wipe-clean surface so they can be updated whenever necessary, and printed symbols and prompts help carers and family provide appropriate information.

We have retained the benefits of our original charts: symbols and prompts to help carers and family provide appropriate information, and a wipe-clean surface so that it can easily be updated as often as necessary. And when the information is not needed, simply turn it to the wall and display instead an attractive picture.

* Pen fixing, velcro strip with adhesive so you can choose where is most convenient for you to store the pen.
* Dry wipe marker pen. We supply a dry wipe marker pen with each chart. You can also use any other brand of dry wipe marker pen.
* Hanging mechanism enabling either side of the chart to be displayed

Not everyone has the space to display our larger charts in a manner to ensure that personal information remains confidential. The Twist-N-View™ range addresses both these issues: the charts are smaller and, in making them double-sided we have ensured that all personal information can remain confidential, yet all a carer needs to do is turn the chart over to access information that will help them provide informed, person-centred and dignified care.

The ‘Display’ version
This ‘Display’ version of the Twist-N-View™ range provides clear plastic pockets for you to insert whatever you wish, for example a selection of photos or further care information, and has space for an individual’s needs and preferences to be written on the reverse. The charts have a wipe-clean surface so they can be updated whenever necessary, and the printed symbols and prompts help carers and family provide appropriate information.

Hang the chart in an individual’s room so that the information a carer needs is instantly accessible, and, when the information is not being required, protect confidentiality by simply turning the chart to the wall and display the individual’s photos or other information instead.

Standard Care Charts – Small

Where limited space prevents the full size chart from being used, this reduced sized version (about half the size of our Standard chart) offers the ideal compromise. It operates in exactly the same way with prompts to help families and carers provide appropriate information, symbols to help carers find the information they need at a glance, and a wipe-clean surface to allow notes to be updated as often as required.

This is a single sided chart supplied with white tack, dry wipe marker pen and velcro pen fixing. If you are looking for the double-sided Twist-N-View range click here.

Each chart is supplied with a dry wipe marker pen, velcro tab for fixing pen to the wall or chart, and white tack for fixing the chart to the wall, back of door or wherever you choose.

White plastic hangers are available for £1.75 each should you prefer to hang your charts on a hook with cord. Add the quantity of charts required to your Cart and then click on View Cart to be offered option to purchase hangers.

Once you have created a Mycarematters* profile, you need to ensure that healthcare professionals are aware of its existence. These three items all have your unique Mycarematters code printed on them, enabling hospital staff to retrieve, print out and display your profile. In this way, everyone helping to care for you will be aware of your needs and preferences.

*Mycarematters is an online service where a person, or a carer / care home on their behalf, can upload the information they want healthcare professionals to know if they are admitted to hospital and are unable to communicate. So if a person has to undergo a stay in hospital and has created a Mycarematters profile in advance, their needs and preferences can be accessed the minute they arrive on the ward. Their Mycarematters printout can also be used to assist with care in a care home or in their own home.

This wallet card has your unique Mycarematters code printed on it, enabling hospital staff to retrieve, print out and display your profile. The reverse of the wallet card has space for you to add contact details of the person you would like to be contacted in the event of an emergency.
